In consideration of being permitted to participate in any way in the 2026 Long Island Warriors Summer Shootout, I acknowledge, understand, and agree to the following:
I understand that participation in ice hockey involves significant risks of injury, including but not limited to serious bodily injury, permanent paralysis, or death. While rules, equipment, and personal discipline may reduce these risks, the risk of serious injury cannot be eliminated and does exist.
I knowingly and voluntarily assume all risks associated with my participation in the 2026 Long Island Warriors Summer Shootout, both known and unknown, including those that may arise from the negligence of the released parties or others. I accept full responsibility for my participation in this event.
I agree to comply with all stated and customary rules, terms, and conditions for participation in the 2026 Long Island Warriors Shootout. If I observe any unusual or significant hazard during my presence or participation, I will immediately remove myself from participation and promptly notify the nearest event official.
On behalf of myself and my heirs, assigns, personal representatives, and next of kin, I hereby release, indemnify, and hold harmless Northwell Health Ice Center, its officers, officials, agents, and employees, as well as other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, the owners and lessors of the premises used to conduct the event (collectively referred to as the “Releasees”), from any and all claims, demands, losses, or liability for injury, disability, death, or damage to person or property arising out of or related to my participation in the event, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
